New Patient Registration

If you would like to register with our practice, and are within the practice area, the online registration form is available below or you can collect a form from the reception desk in the practice.

To gain online access to your medical record, ordering prescriptions and making appointments we recommend you download the NHS App and follow their instruction to register or click here to register via ‘Airmid’.

Reception can also register you for online access. To do this please present to the practice with 3 forms of identification, 1 photo eg passport or driving licence, and 2 with your current address on.
